Our Policies

Check In Policy:

At each visit, we ask you bring a copy of your insurance card and an identification card. We also ask you to arrive 15 minutes before your scheduled annual exam or problem visit. If you will be a new patient with us, we ask that you arrive 20 minutes early.


Financial policy:

All patients are required to pay their copay at each visit for that date of service.

If you are a private pay patient, you will be required to give an $100 deposit for a problem visit and a $200 deposit for a new patient exam or annual exam.   When you pay your bill completely for that date of service, you will receive a 55% discount.


Return phone call policy:

When you call our office, we may not be able to answer your call immediately as we are helping other patients. If you leave a message on the appointment line, the schedulers will call you back during the day you call. Please make sure you leave a phone number that they can call you back promptly on.   If you leave a message on the triage line with a question or problem, the triage will prioritize the calls and will return your call within 48 business hours. If your message is left after 4pm, then the 48 hour return call policy begins the next business day.

Prescription Refill policy:

Please contact your pharmacy for a refill on your medication. Do not call the doctor on call for a routine medication refill after hours or on the weekends.
